Submitting Health Forms

Health Services utilizes PyraMED, a student health portal, to collect your health forms. York College of PA requires your health forms before you can attend.

Instructions:

  • Visit ycp.studenthealthportal.com
  • Log in with your YCP credentials
  • Once you are logged into PyraMED, you will need to complete the following eight electronic forms by clicking on "My Forms" or the "Pending Forms" link. Please read all directions. *Starred items are REQUIRED. Not completing these forms will result in a hold on your account.
    • *Medical History - Upload your Physical Exam Form.
    • *Immunization History - Upload your Immunization Record Form
    • *TB Screening - If you answer “YES” to any of the questions on this form, you will need to complete the Positive TB Screening Form.
    • Emergency Contacts - List someone we can contact in case of an emergency
    • HIPAA Forms - Required to speak with someone other than the student regarding medical care
    • Insurance Card - Upload a picture of the front and back of your medical insurance card
    • COVID Immunization History - Please upload a picture of your vaccination record card
    • Communication Preferences- This is required to send protected messages regarding appointments or private health information

Important Information:

  • If you are a new accepted student, please use the printable forms below:
  • If you have already completed an older version of these forms, you may still submit them. You do NOT need to have your doctor fill out new forms. 
  • Any form or print-out from your doctor's office is also an acceptable form to submit, as long as it contains the information requested by York College's Health Services.
  • If you are an international student, blood tests (titer levels) showing immunity to the required immunizations must be attached/uploaded online under the Immunization history form.
